COMPARISON BETWEEN EXCISION OF BENIGN OVARIAN CYSTS BY LAPAROSCOPY AND BY LAPAROTOMY

ABSTRACT
Background: Benign ovarian cysts represent common health problem and laparoscopy has an important
advantages over laparotomy in the management. Objective: The aim of this study was to compare the clinical
outcome of laparoscopic intervention versus open surgery for ovarian cysts removal. Patients and Methods: An
analytic comparative prospective study was conducted for the period one year (2023-2024) at Tishreen University
Hospital in Lattakia-Syria. The study included women with diagnosis of ovarian cysts, who were managed either
by laparoscopy(30 cases) or by laparotomy(30 cases), in which demographic variables with morbidity were
analyzed and compared between two groups. Results: Patients ranged in age from 16 to 47 years old with an
average age was 30.45±10.1 years. Women who underwent laparotomy were significantly younger(25.6±8.6 versus
35.3±9.1 years,p:0.0001) with lower size of cysts(8.87±1.1 versus 12.03±2.1 cm, p:0.003). Majority of cysts in
laparotomy group were functional (80%) versus(30% )in traditional open surgery group, p:0.0001. There were no
significant differences between study groups in term of location of cysts(p:0.2) and previous history of surgical
intervention(p:0.7). Duration of surgical intervention was significantly shorter in women who underwent
laparoscopic intervention(39.73±9.4 versus 58.92±16.3 minutes in open surgery group, p:0.0001) with lower
perioperative blood loss(38.44±21.3 versus 278.12±59.9 ml, p:0.0001). Total hospital stay was shorter in
laparoscopy group(7.78±1.2 versus 37.64±19.4 hours in other group,p:0.0001). In addition to, return to normal life
was earlier in laparoscopic 7 و 6 و 5 group(8.45±1.7 versus 29.42±5.3 days in group 2, p:0.0001). Conclusion: The
current study revealed superiority of laparoscopy to open surgery in management of benign ovarian cysts regarding
of morbidity as well as lower hospital costs.
