ASSESSMENT OF THE SETON TIE TECHNIQUE IN MANAGEMENT OF PERIANAL FISTULAS

ABSTRACT
Background: Perianal fistula is considered one of the most difficult surgical disorders, and there is a wide range of basic surgical interventions. Objective: The aim of the current study was to evaluate the efficacy of Seton tie technique in management of perianal fistula based on its post-operative outcomes. Patients and Methods: An observational retrospective study was conducted for the period two- years (January 2021- December 2023) at the department of general surgery- Tishreen University Hospital in Lattakia-Syria. The study included 47 cases of perianal fistula who treated by Seton-based technique, in which sociodemographic variables, with final outcome and their associated factors were analyzed. Results: A total of 47 patients, 40 males (85.1%) and 7 females (14.9%), with a mean age of 38.11±10.9 years (range 19-71) were included in the study. 42.6% of the patients were overweight and 23.4% had concomitant diseases represented by hypertension and diabetes mellitus in percentage 12.8% and 10.6% respectively. Majority of the patients (93.6%) showed recovery after the procedure with recurrence rate 6.4%. There were no significant differences regarding of final outcome according to gender(p:0.06), age(p:0.5), body mass index BMI(p:0.8), presence of diabetes mellitus DM(p:0.05) and hypertension(p:0.9). The rate of incontinence after surgery was 4.3%, in which comorbidities and BMI of the patients didn't influence postoperative continence. Conclusion: The current study revealed that Seton tie technique is an effective and safe method for high lying perianal fistula.
