A CLINICAL STUDY TO EVALUATE THE EFFICACY OF AMRUTADI TAILA ANUVASANA BASTI AND DASHMOOLADI NIRUHA BASTI IN THE MANAGEMENT OF POST CHIKUNGUNYA SYMPTOMS W.S.R. TO AAMVATA

ABSTRACT
Chikungunya fever is caused by the chikungunya virus (CHIKV), an arbovirus transmitted by Aedes species mosquitoes. Although chikungunya fever resolves within 5-14 days in the majority of patients, more than 40% develops arthritic manifestations lasting more than 3 months after the acute illness1. Rheumatic features include symmetrical polyarthritis[1] often affecting the hands and feet similar to Amvata in Ayurvedic texts. (Rheumatoid Arthritis in modern medicine). Amavata is an Ama Pradoshaja Vikara, caused by Agnimandya, which leads to accumulation of Ama in Shleshmasthana and Prakopa of Vatadosha[2], resulting in Pain, stiffness and swelling of joints, which closely resembles chronic inflammatory autoimmune disorder called Rheumatoid arthritis.[3] In Post chikungunya symptoms, modern medicine treatment provides symptomatic relief with DMARDs (Disease-modifying anti-rheumatic drugs), Corticosteroids, NSAIDS, etc. drugs used.[4] The underlying pathology remains unchecked because of the absence of effective drugs and drugs which are available are not free from side effects. It shows Adverse GI effects, hepatotoxic, neurotoxic osteoporotic, etc. side effects.[4] Ayurveda has significant and competent treatment for Post Chikungunya symptoms w.s.r.to Aamvata with less side effects and less complications. Ayurveda aims at treating the disease with Shamana (Conservative) and Shodhana (Purification of body) Chikitsa.[5] Ayurveda cures the cause of the disease which leads to the violation of Samprapti of the disease. In current study, various Shamana Aushadhis and Panchakarma process helps in reducing the Post Chikungunya symptoms w.s.r. to Amavata lakshanas. Panchakarma Chikitsa such as Sarvanga Abhyanga, Bashpa Swedana, Patra-pottali Swedana, Amrutadi taila Matra Basti and Vyatasata Niruha Basti with Gokshur, Dashmoola and Guduchi Kwath dravyas for 15 to 30 days according to severity of the symptoms and Shamana Aushadhis for 42 days given to the patients according to severity of symptoms. It is an open clinical study with pre-symptoms and post-symptoms gradation design wherein 10 patients diagnosed as Post Chikungunya Symptoms of either sex was selected. By Using Shamana Chikitsa (Internal medicines) and Shodhana Chikitsa (Panchakarma) in operation, Cases were successfully managed. There has been significant improvement in signs and symptoms after treatment and no problem was found during treatment.
