AN AYURVEDIC APPROACH IN THE MANAGEMENT OF PRAMEHA PIDAKA (DIABETIC CARBUNCLE) – A CASE REPORT

ABSTRACT
Prameha pidaka, known as diabetic carbuncle, is a serious complication of Prameha related to imbalances in kapha dosha, meda, and kleda, as per classical Ayurvedic texts. This case report describes the management of a 60-year-old female with uncontrolled diabetes mellitus, presenting with a painful, suppurative lesion in the epigastric region. Treatment was guided by Acharya Sushruta's approach for pidaka, utilizing surgical intervention (incision and drainage) along with internal and external Ayurvedic medications to control infection, support wound healing, and address systemic imbalances. Medications such as Triphala Guggulu and Gandhaka Rasayana facilitated vrana ropana and raktashodhaka, while kashayas aided in glucose regulation and inflammation reduction. Over a 35-day period, the lesion healed, blood glucose levels stabilized, and the patient’s overall health improved. This case emphasizes the efficacy of holistic Ayurvedic management in addressing both the symptoms and underlying causes of Prameha pidaka.
